Physican Orders for Life Sustaining Treatment (POLST)
The POLST is a physician-signed medical order that tells emergency responders, hospice, and hospital staff exactly what kind of treatment to provide—or avoid—during a medical crisis or end-of-life situation.
This version has been tailored for cryonics members and is adapted to Michigan under the MI-POST system.

Authorizes CPR after clinical death—not to revive you, but to preserve tissue for cryopreservation.

Directs full medical interventions (like ventilation, intubation, and defibrillation) only if they support cryonics preparation—not to prolong dying.

Allows short-term hydration or nutrition if it supports cryonics goals.

Explicitly allows post-mortem procedures like external cooling and anticoagulants to begin immediately.

Signed by your physician, which gives it legal authority and ensures it is followed by EMS, hospitals, and hospice teams.
Unlike a DNR, which often gets misinterpreted, the POLST gives clear and customized instructions for cryonicists—and it’s one of the most powerful tools you can have at the time of death.
NOTE: MI-POST is intended for people likely to die in the near future; it may also have to be renewed periodically.
